Outreach Support

Outreach Support usually means you will be supported by one member of staff that you know who will help you to:

 

    • Do different things in the community
    • Be helped in your home
    • Meet friends and join clubs
    • Go to college
    • Go to work
    • Access  public transport

 

Outreach support is available on week days and weekends between 7am and 11pm with the exception of Bank Holidays and the 27th – 31st of December inclusive

Outreach Support is a service that people can use at a time when they do not need accessible transport.  You are required to pay your own transport costs or use your Freedom Pass, Taxi Card or Mobility Allowance if you have them.  Eastway will provide an Oyster Card for its Outreach Workers but any extra travel costs must be paid by the service purchaser.

On Outreach Support you are required to provide your own lunch.  Outreach Workers also provide their own lunch and may choose to purchase food in a cafe that you have chosen to eat in, or wait and eat take away or a packed lunch after supporting you to eat.  If invited by you to have lunch in a restaurant in which they unable to eat their lunch you must fund the Outreach Worker’s lunch.

On Outreach Support you are required to pay the costs of any activities you choose to attend including those accrued by your Outreach Worker in supporting you.  This includes such costs as concert tickets and entry to museums, leisure centres and night clubs.

Outreach support can help you in your own home to become more independent. Eastway are planning to be able to offer you personal care in your home in the future if you need it.
